The Colorado Immunization Information System (CIIS) is a computer system operated by the Colorado Department of Public Health and Environment under the Colorado Immunization Act. The system helps healthcare providers, schools, daycare providers, health plans and you keep track of the shots you and your family receive.

As of April 19, 2010 Med Peds Clinic of Fort Collins will begin participating in CIIS. The shots your family has already received will be entered into the system. Shots received in the future will also be entered. This information is confidential and is covered by the Health Insurance Portability and Accountability Act (HIPAA). Under Colorado law, anyone who releases information from CIIS to anyone who is not permitted to have the information, commits a crime and can be punished.

You can choose at any time to have you or your family’s shots excluded from CIIS. You need only to request and send in an Opt Out form that you can obtain from our receptionist. For additional information about the registry please visit http://www.cdphe.state.co.us/dc/immunization/ciis/.
